Depois de um ano marcado pela pandemia há razões para crer um 2021 melhor.

O ano acabou e infelizmente os negacionistas estavam enganados.


Tal como era de prever pelos primeiros números de taxa de contágio e letalidade conhecidos, o SARSCov2 mata muito mais que uma gripe sazonal, doença que já de si era motivo de grande preocupação, custos económicos e de vida, todos os anos.
O que era inicialmente especulação racional agora é matéria de facto.
De facto, só em Itália, um único país, o COVID19 matou mais que em toda a Europa a gripe mata num ano (1 e 2) . E a gripe não é nem vai ser uma doença pouco importante a que ninguém dá importância, é uma doença com um peso económico e sanitário grave e preocupante (1).
Em vários países do primeiro mundo, como Itália (3), Espanha(4), Brasil (5) e EUA (6) a COVID19, é a primeira causa de morte. Isto estabelece para lá da dúvida razoável o potencial da doença - e de como sem prevenção e a seu tempo a mortalidade é elevadíssima.
E a mortalidade não é o único problema. Sabemos que há doença prolongada que afeta muitos doentes que precisaram de hospitalização (7),(8),(9), (10) e sequelas (11) mesmo em pacientes de baixo risco (12).

Felizmente, muitas décadas de conhecimento acumulado permitiram desenvolver uma vacina durante o ano anterior ao que agora começa, um pouco do mesmo modo que um ovulo fecundado apenas demora 9 meses a transformar-se num bebé humano, mas demorou milhares de anos até que a evolução o permitisse. Mas aqueles que não temem o vírus e dizem que não é um problema, temem a vacina (que é e faz apenas uma parcela do que o vírus é e faz) em parte porque acharam este processo demasiado rápido, quando demorou um ano a implementar o que já se sabia como fazer.

Se vamos pensar no que não sabemos, só faz sentido preocuparmo-nos com o longo prazo. Mas mesmo no longo prazo, o vírus promete ser muito mais grave que a vacina, através de todas as idades. Porque (como podemos ver acima e nas ligações abaixo) o vírus promete problemas a longo prazo, mas a vacina, não. Porque a vacina não tem a atividade do vírus e é apenas uma parte pouco funcional deste. Apenas tem mRNA para produzir uma proteína, versus o vírus todo. Racionalmente, a melhor conclusão a que podemos chegar é que só será mais seguro não apanhar a vacina, num mundo onde o risco de apanhar o vírus todo é muito, muito baixo mas esse mundo não é um mundo sem prevenção contra COVID19. Esse mundo é um mundo onde a maioria tomou a vacina e conseguiu atingir imunidade de grupo. E isto deverá ser verdade mesmo para aqueles que não farão grande doença com COVID19 - a vacina é e faz apenas uma parte do que o vírus é e faz.
E não, a vacina não tem nada que entre no núcleo das células onde está o DNA e não interfere com este: usa o material de síntese proteica no citoplasma, como o vírus.
Não faz sentido ter medo da vacina e não do vírus.
E se queremos salvar a economia, que muitos dizem que é o mais importante, então este é o caminho. Continuar em negação não vai ajudar.
E não, não recebo dinheiro de nenhuma farmacêutica, e sim, a esperança média de vida tem aumentado desde que existe medicina e farmácia e não, não há maneira de ter um chip 5G com bateria e antena a passar por uma agulha intramuscular.
A motivação para escrever isto é que há razões para que 2021 seja melhor que 2020 em muitos aspetos mas para isso é preciso que as pessoas se vacinem e que o façam compreendendo porquê (não podem ser simplesmente obrigadas).

Consultas de 10 minutos


Em Inglaterra o tempo de consulta médio para humanos ou animais de companhia é de cerca de 10 minutos.Cá as consultas médicas de 10 minutos também são sistema em muitos sítios.  Isto sempre me pareceu muito pouco tempo, e defendo que as consultas devem ser mais longas e com limites flexíveis. Dificuldades com limitação de recursos face a populações crescentes com necessidades cada vez mais complexas podem ter levado a este cenário. Mas as coisas até estão a melhorar ainda que lentamente. O problema é que muitos médicos e médicos veterinários (na prática em Inglaterra) consideram mesmo que este tempo é suficiente. Mas muitos outros nem por isso, então eu penso que temos de olhar para os estudos e ver o que dizem. Eu vou apresentar argumentos para que se aumente o tempo de consulta e que se isso requer mais meios que se lute por eles. A desculpa pode ser económica, mas não é médica.

Permitam-me abordar ambas os problemas (medicina humana e veterinária) de uma só vez, por 3 razões. A primeira é que existem poucos estudos e são quase todos de medicina humana pelo que quero juntar o que se pode de medicina veterinária. Outra é porque o problema é o mesmo. Por ultimo, existem paralelos  na forma de colher uma história, observar sinais e sintomas e chegar a uma lista de diagnósticos diferenciais e estabelecer uma terapia. Isto porque a fisiologia é a mesma, muitas patologias são as mesmas ou tem o seu análogo, a ciência é a mesma e o método é adaptado a cada espécie mas com poucas diferenças. Poder-se há argumentar que existem imensas diferenças e existem, mas parece-me mais difícil argumentar que isso se traduza em grandes diferenças de necessidades temporais na clínica geral. Consultas de especialidade são outra questão.

Como escrevia antes, existem poucos estudos. Mas existem já alguns que deixo em baixo para referencia e consulta dos interessados. De facto, existem razões fortes para defender consultas superiores a 10 minutos. Eu penso que neste momento a média deveria rondar, não os 10 mas a meia hora. Mas isto é mesmo uma opinião, porque a única coisa que posso sustentar é que 10 minutos não chega para a clínica geral, seja de que espécie de mamífero seja. Dos que são capazes de argumentar ou dos que não são.

Em resumo, pelos  resultados dos estudos, posso defender que consultas mais longas têm estas vantagens:

- Mais problemas são discutidos por consulta (e eles aparentemente andam aí)
- Permite fazer perguntas abertas, em vez de perguntas fechadas de sim ou não que podem deixar algo por dizer.
- A relação com o paciente tende a ser melhor numa consulta longa e possibilita construir um ambiente mais positivo que leva a melhores resultados
- O médico pode sentir pressão para cumprir um determinado horário e muitos admitem que isso interferiu nas decisões clínicas.
- Leva a mais consultas de controlo e confirmação do sucesso terapeutico.
- Dar ao doente estimativas de custos e de proporcionar consentimento informado requerem mais que 10 minutos de consulta.
- São detectados mais problemas em consultas mais longas.
- Satisfação do paciente aumenta em consultas longas.
- Pode estar a alimentar a prática da chamada medicina integrativa que pouco mais oferece que tempo.

Parecem-me vantagens substanciais. Do ponto de vista médico parece-me que não deviam deixar dúvidas. Num mundo de recursos limitados, por vezes, pode não ser possível fazer as coisas como se devia, mas esse é outro argumento, para abordar com outras ferramentas.

O monarca vai de fato de banho.

O homo sapiens conhecido como Prince Charles, defende uma série de disparates como práticas médicas e médico-veterinárias. Como por direito de nascença é melhor que os outros homo sapiens, mesmo sem ter feito nada para isso, tem audiência que não acaba. É um disseminador de memes privilegiado, teve um tipo de sorte que só cabe a um punhado dos 10 biliões de habitantes da Terra. Podia fazer diferença, mas  não teve sorte na capacidade de pensar criticamente e compreender ciência básica. Os memes que dissemina são aleijados.

Quer tratar doenças com agua. Não é muito diferente em termos de treta, das roupas do outro rei, que também não tinham nada que as fizesse ser roupas. Umas são ar,  outras são agua. Ar não é roupa e agua não é medicamento. Nem tem memória.

É caso que para dizer que o monarca vai de fato de banho. E a escorrer agua.

O John Oliver tem razão. O jornalismo de ciência é mau. E ainda há mais que ele não disse.

Como diz a "Ars Technica", o "rant" do John Oliver acerca da ciência e da comunicação da ciência é merecido.

A intensidade com que a comunicação social nos despeja com a interpretação mais sensacionalista possível,  de estudos de qualidade heterogénea, descontextualizada e contraditória, é de tirar a qualquer um do sério. E fora do sério mas com toda a razão, o  John Oliver toca na maioria das feridas. Não vou repetir as que ele foca. Vejam o video abaixo. Vou acrescentar apenas umas picuinhices que não parecendo, ainda são importantes.

A replicação, a plausibilidade do estudo, a qualidade do desenho, etc, são tudo coisas importantes. Mas para além disso existe outra coisa que só o tempo traz. A importância de um estudo cientifico está muito ligada ao tipo de evolução que ele permite trazer à ciência ou como dela faz parte. Estudos que integram um plano de investigação progressivo, ou seja, um plano de investigação que vai abrindo portas a novas previsões para testar, e que permite interpretar e passado de um modo mais consistente e completo, são o tipo de estudos que consideramos ciência. São como peças de um puzzle a encaixar e a dizer como deverá ser a seguinte.

Se são estudos, que apesar ou não de integrarem um tema, não levantam novas questões que possam ser testadas (e falsificadas), requerem explicações ad-hoc, são incompletos e inconsistentes nas explicações que trazem, e as anomalias só se acumulam, isso é pseudociência. Como um todo, esses estudos são planos de investigação degenerativos.

Esta critica à apreciação de um estudo e do seu valor, é um passo acima da que vem da máxima implícita no discurso do John Oliver de que "não é ciência até ter sido replicado". E isto é uma boa máxima, já que em média um estudo é refutado em cerca de 6 meses (a replicação normalmente não vem nos média e nem na comunidade cientifica tem muita divulgação e de facto não é sempre tentada, já que "vale pouco mérito"). Mas  um estudo tem de facto de ser replicado para ser levado a sério e normalmente se é algo muito importante, alguém tenta replicar ou esmiuçar o estudo até ao mais pequeno pormenor. A comunidade cientifica compete por recursos e não perdoa o disparate por aí além. Mesmo assim, a não replicação de estudos ( ou a sua não divulgação)  por falta de mérito e recursos é de facto um problema por resolver. É preciso mais.

Mas acima de tudo, para que as suas conclusões - de um determinado estudo -  sejam aceites para além dos factos (que até podem ser replicados embora a conclusão esteja errada), tem de pertencer a um plano de investigação progressivo e não degenerativo.

Isto é o que dizem os filósofos da ciência, originalmente por Lakatos, que tem a proeza de resolver os problemas não resolvidos por Popper de uma maneira muito satisfatória e que permite sustentar que ciência é o conhecimento que é feito das melhores explicações possíveis. Quando deixa de ser a melhor explicação possível é porque é preciso encontrar outra. Isto é o que os cientistas têm andado a fazer. Ou a tentar fazer. Com um sucesso bastante porreiro.

Outra coisa que o John Oliver não toca, é a estupidez dos documentários apresentarem todas as ideias dispares acerca de um tema com a mesma "neutralidade".

Dão o mesmo tempo de antena aos vários intervenientes, o mesmo ênfase (e aqui já é quando temos sorte), e depois o pessoal que tire as suas conclusões. Está errado. Leigos, por definição,  não estão preparados para comparar determinados argumentos intuitivamente. E está a ajudar a ter uma sociedade com dois pesos e duas medidas - um para a ciência (mais exigente) e outro para a treta (nada exigente). Existem coisas que são matéria de especialista, cuja argumentação completa ultrapassa qualquer documentário de poucas horas e não pode ser apresentada como se a opinião de cada um pudesse ser apresentada ao lado de um trabalho metódico e auto-correctivo. Ao menos que promovam debates sérios onde as tretas possam ser confrontadas ponto a ponto por quem tem a capacidade de o fazer.

E agora o John Oliver. É do melhor que anda por aí. E sim as TED talks são "hit or miss".

Actualização sobre a acupunctura.


E essa justificação tem de incluir tudo o que se sabe acerca da realidade, e naturalmente do que é pertinente para avaliar a afirmação. Por isso temos de considerar a plausibilidade à priori antes de fazermos qualquer teste. Senão, estamos a descartar tudo o que já foi estabelecido como conhecimento como se não valesse nada, e isso não faz sentido. A matemática que permite fazer cálculos usando a plausibilidade de uma afirmação perante um determinado teste,  ou seja, a probabilidade à priori, é a estatística bayesiana. Porque testes positivos aparecem sempre que  se repetir uma experiência vezes suficientes, e não é por isso que se manda tudo o resto que se sabe às urtigas. A ciência funciona porque aposta na consistência, não na quantidade de explicações que aceita, só porque alguém acredita ou porque tiveram sucesso em poucos testes, testes mal desenhados  e escolhidos apenas entre os que deram o resultado que se queria (vulgo "cherry picking"). Por isso se diz que "afirmações extraordinárias requerem provas extraordinárias".

Então, decidi ver qual era o estado da atual pesquisa cientifica sobre a acupunctura. E parece que até os mais exigentes investigadores andam à procura de "qualquer coisa onde isto possa ser útil" dado a quantidade de coisas em que têm tentado avaliar a sua eficácia. Mas o panorama é o esperado e de acordo com as conclusões anteriores. Os testes são mal desenhados, insuficientes, e não permitem concluir eficácia excepto nalguns casos que sugerem, na mesma, ser pouco significativo. Neste cenário, e perante a falta de plausibilidade cientifica, a probabilidade que devemos dar à eficácia da acupunctura é perto de nenhuma. Ou seja, pouco mudou no mundo cientifico quando à validação da acupunctura desde que a investiguei a ultima vez.

Dado o resultado dos estudos sistemáticos (e revisão das metodologias dos testes) em face de tudo o que se sabe sobre a realidade, a a minha conclusão é: A acupunctura continua a ser pseudociência e não tem lugar na medicina. A minha justificação é: A acupuncura não prova ser capaz de ter um efeito significativamente superior ao placebo na esmagadora maioria das situações estudadas, não apresenta uma justificação cientifica, e  leva a um campo de investigação regressivo  onde continua a ter de ser defendida por evidencia escassa, anedótica e baseada na popularidade e crença pessoal.

A humanidade viveu baseado nesse tipo de conhecimento durante 200 mil anos em que pouco ou nada evoluiu. A escrita permitiu o nascimento de uma forma de registo e divulgação, e o nascimento da ciência aconteceu sob esta nova aptidão, passado mais uns milhares de anos, um pouco por todas as civilizações, embora muitas vezes apenas pontualmente. Há cerca de 600 anos, na Europa deu-se a revolução cientifica. O poder que nos trouxe foi incomparável com o que tínhamos até agora. O mundo entrou em rápida mudança. Quando é usado para o bem permite as longevidades actuais tão elevadas, a baixa mortalidade infantil e até a erradicação de algumas doenças. Permite que as classes médias vivam melhor que lordes medievais. O caminho a seguir é claramente outro do que o da pseudociência.
